For us to ensure your child's Baptismal date, we must receive the following:
A completed Baptismal registration form
Child's birth certificate.
Baptismal Offering - $200 (suggested offering)
Letter of Permission from the parents' current parish (if applicable)
You will be asked to choose two Godparents' sponsors. They must be a male and a female. Both sponsors must submit a "Sponsor certificate from their parish.
Registration form, birth certificate, consent form (if applicable), parish permission (if applicable) and church offering must be submitted to schedule a date for baptism.
Godparents:
Must have the ability and the intention to fulfill the role of Godparent and will be required to submit a "Sponsor Certificate" from the parish they attend.
Must be a least 16 years of age.
Must be a confirmed Roman Catholic and leading a life of harmony with the Catholic Faith.
If there is at least one Roman Catholic Godparent, a baptized person who belongs to another Christian community may be admitted as a "Christian Witness" (not a Godparent). A non-baptized person cannot be a witness of a Godparent.
Parents preparation:
Parents are required to attend a Pre-Baptismal Instructional Class before your child is baptized. Classes usually takes place on the 3rd Tuesday of the month at 7PM in the Pastoral Center/Rectory at Our Lady of the Sacred Heart, or it can be scheduled with Deacon Rob Pang at St. John, or it can be scheduled in Spanish with Alex Aguilar or Natalia Lemus at St. John. Parents who are unmarried or not married in the Roman Catholic Church, will need to speak with a priest before the Baptism can be scheduled.
